Important Payment Information
This form must be submitted by the end of the month for which you are claiming payment. Payment for each month's work will be made on the 7th of the following month. For example, work completed in September will be paid on 7th October. If you do not submit your form before payroll is processed, your payment cannot be included and will be carried forward to the following month's payday. Please ensure your submission is accurate and completed on time to avoid any delay in payment.
